Paperwork Rates

Work /School Accommodations - $40.00/  per occurrence

Disability Forms - $40.00 / per occurrence

FMLA Forms - $40.00 / per occurrence

Letters - $20.00 and up

Adoption Paperwork fees may vary.

Insurance 

Services provided maybe covered in full or in part by your health insurance. Please check your coverage carefully by asking the following questions:

• Do I have mental health insurance benefits?
  
• What is my deductible and has it been met?
  
• How many sessions per year does my health insurance cover?
  
• What is the coverage amount per therapy session?
  
• Is approval required from my primary care physician?
  

Our office has a dedicated billing personnel to care for your needs. Please email billing@dbdtllc.com for any questions. 

Payment

Cash, check, FSA, HSA and all major credit cards are accepted for payment.

Cancellation Policy 

If you do not show up for your scheduled  appointment and you have not notified our office at least 12 hours in advance, you will be required to pay a fee of $20.00. Insurance will not cover these fees; therefore, you will be responsible. Short notice cancellation fees are due at the beginning of your next session.
